What You’ll Do

Complete patient assessments and develop individualized care plans
Coordinate services with physicians, therapists, caregivers, and community resources
Monitor patient progress and adjust care plans as needed
Educate patients and families on medications, treatment plans, and wellness
Keep accurate clinical documentation and maintain regulatory compliance

What You’ll Need

Active Minnesota RN license (required)
Home health, case management, or transitional care experience (required)
Strong assessment, communication, and care coordination skills
Must be at least 21 years old
