What You'll Do

Conduct patient assessments and develop individualized care plans
Coordinate services with physicians, therapists, caregivers, and community resources
Monitor patient progress and adjust care plans as needed
Educate patients and families on medications, treatment plans, and wellness strategies
Maintain accurate clinical documentation and regulatory compliance

Qualifications

Current Minnesota Registered Nurse (RN) license required
Home health, case management, transitional care, or related clinical experience preferred
Strong assessment, communication, and care coordination skills
