What You'll Do:

Assess client needs and develop individualized care plans to ensure optimal health outcomes
Coordinate with physicians, therapists, and caregivers to implement and monitor these plans
Educate clients and their families on disease management, medications, and care strategies
Ensure compliance with medical standards and maintaining accurate client records

What You'll Need:

Active MN Registered Nurse License
Minimum of one year of home care experience
Minimum of two years of nursing experience
Must have a valid driver’s license and auto insurance
