What You'll Do:
- Assess client needs and develop individualized care plans to ensure optimal health outcomes
- Coordinate with physicians, therapists, and caregivers to implement and monitor these plans
- Educate clients and their families on disease management, medications, and care strategies
- Ensure compliance with medical standards and maintaining accurate client records
What You'll Need:
- Active MN Registered Nurse License
- Minimum of one year of home care experience
- Minimum of two years of nursing experience
- Must have a valid driver’s license and auto insurance
