What You’ll Do:

Develop and manage personalized care plans to support optimal health outcomes
Collaborate with physicians, therapists, and caregivers to ensure quality of care
Provide education and emotional support to clients and families
Keep accurate, timely documentation in compliance with state and agency standards

What You’ll Need:

Active Minnesota RN License
At least 2 years of nursing experience, including 1 year in home care
Valid driver's license and auto insurance
A heart for people and a desire to provide exceptional care
