What You'll Do:
- Assess client needs and develop individualized care plans to support optimal health outcomes
- Coordinate with physicians, therapists, and caregivers to implement and monitor care plans
- Adjust care strategies as needed based on client progress and changing needs
- Educate clients and families on disease management, medications, and care best practices
- Maintain accurate records and ensure compliance with all medical standards and regulations
What You Will Need:
- Active MN Registered Nurse License.
- Experience in home health, case management, or clinical care (Preferred).
