How you will make an impact:
As an RN Case Manager at Baywood Home Care, you will be responsible for assessing client needs and developing individualized care plans to ensure optimal health outcomes. Your role involves coordinating with physicians, therapists, and caregivers to implement and monitor these plans, making adjustments as necessary. You will educate clients and their families on disease management, medications, and care strategies, while also ensuring compliance with medical standards and maintaining accurate client records. Your efforts will be pivotal in supporting client recovery and promoting overall well-being.
Schedule: This is a full-time position working Monday-Friday 9:00am-5:00pm.
What you will need:
- Active MN Registered Nurse License.
- Minimum of one year of home care experience.
- Minimum of two years of nursing experience.
- Must have a valid driver’s license and auto insurance.
