What You’ll Do
- Complete patient assessments and develop individualized care plans
- Coordinate services with physicians, therapists, caregivers, and community resources
- Monitor patient progress and adjust care plans as needed
- Educate patients and families on medications, treatment plans, and wellness
- Keep accurate clinical documentation and maintain regulatory compliance
What You’ll Need
- Active Minnesota RN license (required)
- Home health, case management, or transitional care experience (required)
- Strong assessment, communication, and care coordination skills
- Must be at least 21 years old
